This step-by-step tutorial is designed to guide you through the process of creating a new form module

Instructions:

Navigate to the Module Options

• Go to the side-menu and locate the "Module" button.
• Within the module menu, choose the "Forms and Surveys" button.
• A new page "Forms and Surveys" will open.

Make a New Form Module

• On the right side of the page, click on the "New Form and Survey Module" button.
• Enter the Module Name, Call to Action, and, if desired, add a description to the form in the "Start Screen." Alternatively, you can start your form with questions without any introductory description.

Craft Your Questions

• Select your question type from the various options available. Write your question in the "Question Text" box. Additional information can be included in the “Subtext” box.
• For specialized question types like drop-down or multiple-choice, customize the choices accordingly. Images and other question types can also be incorporated.

Save Your Form

• Click the "Save" button to preserve your form within your experiences.

Integrate the Form into an Experience

• Return to the initial side menu and click on the "Experiences" button.
• Open any experience of your choice.
• In the next pop-up menu, select "Build Experiences."

Add the Form Module

• Locate the icon labeled "Forms & Surveys" and drag it to any position among other listed buttons.
• Click on the "Module Name" and, in the drop-down menu, add the form you've just created.

Save Your Changes

• Click the "Save" button to integrate the form into your chosen experience.
